Main Office software version 6313 dated 05/02/18 adds an additional data entry field in Patients/Insurance/Insurance Companies tab for the New Medicare ID. Agencies may begin to populate a patient’s new MBI number as they receive them. At the time point where the agency transitions a patient from billing and OASIS Submission with the old Medicare ID to the NEW Medicare ID (MBI) the new number is available to move to the Primary Insurance ID field and M0063 Medicare ID field on the Patient Demographics screen.

A new Extended Version of the Discharge Summary/Transfer Summary documents are available with Main Office Software version 6294 and Clinical Point of Care version 2681 in the Managed Hosting Environment only. Server based Clinical Software users will not see the enhanced version but may utilize the Discharge Summary/Transfer Summary Enhanced Version in the Main Office Software.
The Extended version has been made available in advance of finalization of the Federal Register proposed rule for Home Health Discharge planning, Discharge Summary/Transfer Summary content requirements.
Upon adding the Discharge Summary record, the option to convert it to a Transfer Summary continues to remain available. Continue to review and complete the dates the Discharge/Transfer Summary covers. Entry of the Date range will be important in utilizing efficient population of the patient history for the document. An additional Check box is available in the upper right corner to utilize the Extended Version. The Extended Version will display additional tabs providing the opportunity to complete the additional content. Each tab provides an option to Import data from the Patient’s Historical Clinical record entered into the software.
The Status tab will import documentation from the Plan of Care to include the patients Functional Limitations, Mental Psychosocial Behavioral status, Allergies and Immunization Status.
The Vitals tab will allow import the patient vital signs documented in the patient’s clinical records for the dates covered by the Discharge or Transfer Summary.
The Wound tab will allow import of wound documentation in the patient’s clinical record for the dates covered by the Discharge or Transfer Summary.
The Meds tab will allow import of the Patients current medications at the time of Discharge or Transfer.
The Illness tab allows documentation of the course of the patients illness. An option is available to import the narrative note from the last or previous Comprehensive Visit assessment.
Consultations tab will allow import of the documentation found in any Communication Notes designated as a Special type, Addition of MD or Physician Referral.
The Labs tab will allow import of the documentation found in any Communication Note designated as Lab/Lab Work.
The Support Tab will allow Import of the documented Supportive Assistance from the Plan of Care.
Orders tab will allow import of all documented Physician Order updates during the dates covered by the Discharge/transfer summary.
The Transition tab allow direct data entry of the Recommendations Instructions and precautions for ongoing care.
The final Tab, Goals will allow import of the Patient goals. After Import, direct data entry of the patient status for each goal is allowed.
The Final print copy of the Discharge Summary will include patient demographics, Code Status, Language Preference, Diagnosis and a Summary of all services received during the dates covered by the Discharge or Transfer summary.

The training video for the Interactive Plan of Care Guide is available.
A new feature, not included in the training video, has been added to the Comprehensive SOC, Recertification and Resumption of Care Assessments for those agencies utilizing the per visit/ nursing interventions available for initiation through the Interactive Plan of Care Guide. A new Import Interventions button is available in the Comprehensive assessment with Clinical Point of Care software version 2643 dated 12-20-17. The button can be found in the Orders/Treatment section of the Comprehensive assessment, Notes tab. The interventions must be initiated/posted and updated with patient specifics through the Interactive Plan of Care Guide prior to posting in the Order notes tab. The Interventions displaying in the Order/Notes tab will be included on the comprehensive assessment print form.

The Current Treatments screen historically available in the Main office Software in Patients/ Therapies/ Current Treatment tab is now available in the Clinical Point of Care Software in the Managed Hosting Environment ONLY with Clinical Point of Care Software Version 2616 dated 10/02/17. The Treatments tab can be accessed from the Therapy button in the Patient Medical information area.

A new training video on the new Conditions of Participation is now available for viewing. The training video will review software enhancements to assist agencies in meeting the new Conditions of Participation requirements for Care Planning and Coordination, Patient Rights, Supervision to be implemented January 2018 and Infection Surveillance to be implemented July 2018. Agencies will need to have Main Office Software version 6202 dated 09/20/17 to receive the ICD 10 code set update. No additional action is needed in the main office software or Clinical Point of Care software users in the Managed Hosting environment. Server based Clinical Point of Care users should ensure that their software is updated to the current version and will need to synchronize the Clinical data through the Transmission and Download process. The ICD ‘check’ screen available in the patient medical information and 485 diagnosis screen has been updated to reflect new diagnosis and invalid ICD 10 codes as of 10/1/2017.

Agencies will need to have Main Office software version 6201 and Clinical Point of Care Software version 2611 dated 09/19/2017. Supervisory visits dated 10/01/2017 or later will have enhanced data entry fields. The Supervisory visit will contain three tabs for documentation, Patient response to care, Performance of the Staff person and updates to the Plan of Care. Questions have been reformatted. Additional data entry fields required by new Conditions of Participation effective Jan 2018 have been added. Warnings are also in place for blank responses to items designated as required in the new Conditions of Participation.

Emergency Preparedness training video is now available for viewing. The training video will review software enhancements to assist agencies in meeting Emergency Preparedness requirements to be implemented Nov. 15 2017. Main Office software version 6187 dated 08/17/17 and Clinical Point of Care software version 2601 dated 08/17/17 provide updated data entry fields in the Emergency Contacts screen to indicate which Emergency Contact is a Designated or Legal Representative through check boxes for POA, Durable Attorney for Health Care (DPOAHC), or Patient Representative. New Conditions of Participation require Notice of Patient Rights to be provided to the patient Legal Representative and any designated patient representative. Agencies may now create a Communication Note in both the Main office or Clinical Point of Care software with a Special Type of ‘Notice of Patient Rights’. A button will then display ‘Import Notice Copies’. When selected the software will import all Emergency Contacts marked as the POA, DPOAHC or Patient Representative with entered address as needing mailed copies. The Main office can track these Communication notes through Listing/Communication Listing or through the Control Board/PT Activities/ Communication tab. Print Control provides the opportunity to Print the ‘Notice of Patient Rights’ Communication note for individual with address where Patient Rights will need to be mailed. Tracking is available through Print control for recording the date sent and, in the event the Rights are needed returned with a signature, a Return Date.

Clinicians utilizing the Clinical Point of Care Software in the Managed Hosting Environment may now monitor responses to Notifications sent from the Clinical Point of Care Software from the ‘My Notices’ screen with software version 2582 dated 06/05/2017. In ‘My Notices’ Clinicians will select the new button, ‘Notifications sent’. All notifications sent from a patient’s Control Panel or Authorization Board/Notification section will display in the Notifications sent screen providing efficient monitoring for the receiving clinicians responses. Clinicians may also create a New Notification from ‘My Notices’ by selecting the ‘All Notes’ button. Case Managers may also create a group notification for their Team, as established in the Main Office Software under Employee Teams. Hospice personnel may send group notifications to update the Hospice patient’s Interdisciplinary Group/Team of changes or for notification of patient death.
