DC Medicaid Physician Order Form and Order Form for Long Term Care Services and Supports

DC Medicaid Physician Order Form and Order Form for Long Term Care Services and Supports are now available with Main Office Software Version 6196 dated 09/06/2017.
Agencies will need to select a Special Type when a Physician order is added. Two new options are available, DC Medicaid Services to generate the Physician Order Form and DC Medicaid LT Services to generate the Physician Order Form for Long Term Care Services and Supports. Once the special type is selected the software will display the required data entry fields for completion. Print Forms can then be generated.
