Interactive Plan of Care Guide Software Enhancements and Training Video are now Available

A training video is now available for download titled, Interactive Plan of Care Guide. After the software version update released at the end of the business day 12/18/17, the previously released training video Intervention Guide will no longer be available.
Software enhancements have been made to the Intervention guide feature necessitating a name change. The Interactive Plan of Care Guide is a powerful tool to assist your agency in meeting the new Home Health Conditions of Participation requirements and long standing clinical note requirements. The Interactive Plan of Care Guide fosters development of patient specific interventions, orders, education and training, outcomes and goals using an evidenced based approach. The Interactive Plan of Care Guide uses Best practice interventions with embedded interventions to promote patient engagement through motivational interviewing. Use of the Interactive Plan of Care Guide is an efficient tool for documentation of services performed, patient response or change in behavior to previous services performed, addressing patient barriers to care and evaluation of patient self management. Interventions are provided as a guide. All Interventions should be updated as needed to reflect each patients specific needs, disease process and goals.
All interventions, Orders, Teaching and Goals can be updated for patient specifics.
