Medicare PPS Billing Rejections

Medicare has now starting rejecting claims where your agency zip code does not include the full 9 digits.  In early 2012 we starting adding ‘0000’ to the zip code if the +4 was missing.  As of recently, they are no longer accepting the ‘0000’.  You should also look at any office addresses in “Other Setup” to make sure they also include the +4 digits.

Payroll FICA Social Security Bug

If you do not have software version 5162 or higher, when you generate payrolls for 2013 the FICA social security may calculate at 4.2% instead of 6.2%.  Sorry for any inconvenience this may have caused you.

Year End 2012 Updates

Version 5159 is now available for downloading.

First, this update includes 2013 federal tax calculations which are temporary until the congress determines what the tax rates should be.  Federal income tax will be withheld at the 2012 rates until further notice.  From what we can guess, the FICA SS will return to the 6.2 % rate.  When a final determination is made we will issue an update for both of these.  The payroll system automatically tries to correct the year-to-date FICA SS on each payroll.  If the rate turns out to be 4.2% then the next payroll after your update will correct the year-to-date on that payroll, then take out 4.2% on future payrolls.

Second, this update includes OASIS version V3413 along with the appropriate grouper logic.  The main changes is the addition of some 173## diagnosis codes now included in diagnosis category 3 = diabetes.

Clinical – Error message “xPatients3” during transfers.

If you have done a recent clinical update and are getting an error message referring to “xPatients3” or “xxxPatients3” during downloading your most recent patient data you will need to perform the following:

Update the clinical software to version 2123 or higher. Make sure you open the clinical software to make sure it is version 2123 or higher before you attempt to download your most recent patient data again.  This step not only makes sure the version is 2123 or higher, but also is performing a software adjustment to the clinical data transfer utility program.  If you do not do this step you will continue to get the same error messages.

The error is due to changes we are making to the clinical laptops in order to expand patient demographics information for the future.

Insurance Company Billing Rate Changes

On tonight’s update will be a new tab under insurance companies labeled “Billing Rates.”  This tab will allow the user to more easily change billing rates for an insurance company.  On the tab will be, grouped together, all of the insurance company override rates for that insurance company that were setup under services.

October 2012 ICD9 Code Update

The October 2012 ICD9 diagnosis and surgical codes update will be available on future software updates downloaded as of  Thursday, October 4, 2012.  Once you download your update, simply  go into Utilities, Update ICD9/Proc. Codes, and perform the update.

Medicare PPS Billing

As of 10/1/2012 Medicare will no longer data to appear in PPS billing segments SBR03 and SBR04 in loop 2000B.  These would normally be the Group Name and Insurance Group Number found in Insurance Maintenance and would be rarely used for PPS billing.  We will put out an update tonight that will make sure these elements are always empty for PPS billing even if something is the above fields.
