A new “Symptom Follow-Up Visit (SFV)” option has been added to the Routine Hospice Assessment.
When this is selected, it will open up a SFV Visit tab.
Within the SFV tab, it will display the J2051 documentation from the previous HOPE admission Assessment, HUV1 or HUV2. It will allow for your clinicians to document the symptoms during the SFV visit. This documentation is only for comparison. Your agency process will still have to include adding this information to the original HOPE Admission assessment, HUV1 or HUV2.
Once assessment is saved, it will display SFV after the Hospice Nurse Assessment on the Activities Page, if the Symptom Follow-Up Visit Is selected within the documentation.
This will be available on/after 7/24/2026.
