System Update

In the past when performing a system update, you would be prompted if you wanted to do a precautionary backup. We have removed the feature from the software. Very often a problem would occur during the backup, usually caused by another used getting into the system during the process. In addition, some agencies were relying on this as being their only backup which we do not think is a good practice. We are sorry if this causes any inconveniences.

PPS Therapy Adjustments

Our calculation of the PPS therapy adjustment for final claims was off a small amount. There year-to-year factor that we used to calculate from the original base year on therapy adjustments was not correctly applied. This error had no bearing on actual payments but would show up when the final bill was paid and the invoice the difference had to be adjusted. This has been corrected in version 4793.

New Clinical Release

This week, probably Wednesday, we will be releasing our new clinical laptop software that has been in the works for several months. The backbone of the current clinical software was not designed to handle the load of work and the number of input screens needed as the demands for information & functionality grew over the past five to six years. Unfortunately, we have not been able to meet the needs for additional input screens for Hospice and other areas, until this new clinical software was rewritten.

We have tried to make the new clinical software input screens look as much as possible like the input screens of the old clinical. What has mostly changed are the interface buttons used to access and exit different sections of assessments and activities. The biggest change will be in the patient screen where you will see a notebook tab at the top of the screen labeled “Activities.” All activities for a patient will be added and accessed from this screen instead of different buttons on the right side of the patient screen for the different types of activities. We don’t think these changes will be hard to grasp.

Once inside an assessment, the input screens will work the same as they would if you were doing a comprehensive assessment in the main office software. These new clinical software screens have been in place for several months in the main office software with very few problems.

With the new clinical software in place we plan to finish several Hospice screens and printouts hopefully by the end of May. Once Hospice is done, we will be begin work on several other new activities that agencies have been requesting.

Face to Face Reporting

In the PPS assessment report found in billing, we have added an additional type of report called Face to Face Information. It will show the face to face date entered into the PPS billing information screen. This variation of the report also looks to find a face to face 485 or a face to face physician order that has a matching SOC date and shows the face to face encounter date on the document.

Currently there is nothing in the system requiring a face to face date to be present for PPS billing. The face to face encounter date on the PPS billing screen is more for information purposes. Users can enter the face to face date in the PPS Billing Information screen. When in PPS billing and you select to edit the PPS billing information there is a field where you can enter the face to face encounter date. To help the billing person determine what the face to face encounter is, the document review area will show the face to face date noted on the applicable physician order or 485.

PPS Billing – PPS Billing Rejection – 7202 Missing Admission Type

Agencies are reporting that PPS claims to CMS are rejecting for this reason. We have made a change to PPS electronic billing to include the admission type in electronic claims. We will default to a ‘3-Elective’ if the admission type is blank so that the claims will not reject. In the PPS Billing Information screen there is a new field for the admission type which will override the ‘3’. Version will be 4770 or higher.

OASIS Alerts

There is a new button on the main screen labeled “OASIS Alerts.” This option can generate a listing showing billable PPS OASIS assessments with more than 10 (default) therapy visits, OASIS assessments within 8 (default) days of end of episode and OASIS assessments over 20 (default) days old that haven’t been sent to the state. The report default parameters for the number of days or visits can be changed. Eventually, if users find the report useful, we will create an alerts file that will alert users when they enter the system (especially clinical). We would appreciate response from the users.

Face to Face

Just an update on where we are with the face to face encounters:
(1) The face to face encounter can be done as part of the new generic 485. Inside the 485 just check the “Face to Face Encounter Applies” and it will incorporate into the body of the 485. You can also indicate that you want the face to face to be a seperate document or that the face to face physician is not the same as the 485. If the physician is different, the face to face will automatically change to a seperate document. The face to face document only works on the new generic 485. We will probably not be making any more future enhancements to the older style of 485.
(2) In the physician orders we have added two new order type, “Face to Face Encountere” and “Hospice Face to Face Encounter.” With both of these types some attestation and certification remarks will appear on the printout. For the Hospice Face to Face Encounter you can also specify that the Hospice Nurse Practitioner.
