New Feature for Clinical Software

A new feature has been added to the HHC3000 Clinical Software.  Clinicians, listed as the Responsible Employee for a patient, can now be notified of when the current recertification period ends when they login into the Clinical software.  This feature is available with Clinical version 2997 (06/29/2023) and Main version 7319 (06/29/2023).

Therapy Supervision

An enhancement has been made to the software to allow for documentation and tracking of Therapy (PTA/COTA) Supervision with Main Software version 7287 and Clinical version 2991.

Reactivating Reason Code 31755

Medicare announced that as of 1/3/2023, the reason code 31755 has been reactivated. This means that revenue code 0023 line-item date of service MUST match the date of service for the first home health visit on the claim.

For Initial periods of care, the HHA reports on the 0023 revenue code line the date of the first covered visit provided during the period.

For Subsequent periods, the HHA reports on the 0023 revenue code the date of the first visit provided during the period, regardless of whether the visit was covered or non-covered.

Some home health claims are being returned for Reason Code 31755. The majority of the claims being returned are provider billing error and need to be corrected.  In Currently HHC 3000, agencies will just needs to use the ‘Wrench” button next to the RAP 1st Billable Visit box within the OASIS Billing Info Screen. This will automatically pull in the first billable visit and pull to your claim.

Identified OASIS E Issues–Resolved

There was an issue with C0100-C0500 on the XML being rejected. This issue was resolved with Main software update 7225 and Clinical 2956 dated 1/10/2023.

Identified OASIS-E XML Issue -RESOLVED

We have identified that there is an OASIS E XML issue causing rejections.

The Issues include items D0160, A2121 & A2122A-E, & N0415Z1.

We are working to resolve this promptly. Update will be posted once resolved.

The issues with the XML rejections for the OASIS E questions identified above and also for A1250, K0520, and O0110, have been resolved with Main Software version 7222 and Clinical Software version 2955.  We apologize for any inconvenience this may have caused.

OAISIS E

OAISIS E is available for HHC3000  Main software version  7212 dated 12/28/2022 and Clinical software version 2948 dated 12/28/2022 for OASIS assessments dated 01/01/2023 and/or with a M0090 date of 01/01/2023.

Per CMS OASIS User Manual :   “Please note that the effective date for OASIS-E is January 1, 2023. The version of OASIS that should be collected will be based on the M0090 – Date Assessment Completed. The M0090 date is the last date that information used to complete the comprehensive assessment and determine OASIS coding was gathered by the assessing clinician and documentation of the specific responses was completed.

CMS wants to make clear that with this transition to OASIS-E, there is no need for the use of artificial M0090 dates. All assessments with a M0090 – Date Assessment Completed on or before December 31, 2022, including the last 5 days of 2022 must be completed with OASIS-D1.  This is true even when the first day of the new certification period is on or after January 1, 2023.

All assessments with a M0090 – Date Assessment Completed on or after January 1, 2023, must be completed with OASIS-E. This is true even when the assessment was initiated in 2022.”

New ICD 10 Code Set Effective 10/1/2022

As of 10/1/2022, agencies must ensure they are using Main Software Version 7169 or later and Clinical Point of Care Software Version 2930.

These versions will ensure that agencies will receive the updated ICD 10-CM code set, the new grouper Version for OASIS due to the ICD 10-CM code set update, and the Hospice Wage Index.

Currently anyone logging onto crystal.hhc3000.com may experience some issues with logging on. This is a known issue and our technical staff is looking into it.

New PDGM Statistics Summary Report

A new benchmarking report has been added to HHC3000 to assist in analyzing PDGM data. This tool is designed to aid in decision making and assess your company’s  performance with real time data.

The PDGM Statistics Summary report is found within the Billing tab under PGDM Reports.

This report is available in the latest update of the Main Software (7113) dated 05/05/2022
